Under the supervision of the Food Services Supervisor, the Cook is responsible for the preparation and service of well-balanced meals to residents of the Carefor Civic Complex in Pembroke, ON, following established menus while considering therapeutic diets and resident’s preference in consultation with the supervisor. Responsibilities will be performed in accordance with food handling and safety standards, Canada’s Food Guide, and within the framework of Carefor Health and Community Services policies, procedures, and standards.

This is a part-time, permanent (37.5 hours or 0.5 FTE) position with Allied Health working within the community of Ottawa, ON.
Is responsible for the provision and management of cost effective Social Work Interventions to
individuals and their families in the community. Care is provided within the framework of quality & risk
management and the standards and scope of practice set by Carefor and the provincial regulatory
body (OCSWSSW) for the Social Worker. Is responsible for the assessment, intervention,
consultation and counseling to clients and their families in the community by; maximizing social,
emotional and cognitive functioning; assessing capacity to manage finances and personal care needs,
assisting with obtaining alternative housing; assisting with the establishment and maintenance of
support networks and assisting with crisis management and development of coping strategies.
In all Carefor programs, the Social Worker provides service consistent with the
standards established by Carefor and provincial regulatory agencies/bodies. The
standards of practice include: assessment, planning, implementation and evaluation.
a) Provides complete, relevant assessment of the client and their environment
utilizing pre-set procedures
b) Develops a care plan with the client and family based on the assessment
c) Incorporates discharge planning into the care plan
d) Implements strategies or interventions according to the established care plan
e) Understands and practices role in relation to Standards of Practice for Social
Workers and Carefor policies and procedures
f) Describes, evaluates and documents client’s responses to care plan utilizing
established procedures
g) Utilizes appropriate interviewing and communication skills to obtain information
from the client/family
h) Describes and documents client care activity clearly, concisely, accurately and
appropriately, according to Carefor standards
i) Evaluates client’s/families responses to care plan for the achievement of
realistic client-centered outcomes and revises plan as required
j) Demonstrates evidence of negotiation with client/family during delivery of care
k) Demonstrates respect for client’s choices and decisions
l) Informs clients of foreseeable risks as well as rights, opportunities and
obligations associated with the provision of Social Work services
m) Analyses and incorporates current research findings into practice
n) Reports and discusses client's status on a regular basis with other members of
the health care team as appropriate
Licensed with the College of Social Work and Social Services of
Ontario
• Masters Degree in Social Work Preferred
• Current certification in CPR
• Current and valid driver’s license
• Access to vehicle that is insured for business use
Experience:
